The Background Screening Disclosure and Authorization (FRM-003) is used to obtain an individual’s written authorization for background screening in connection with employment or ministry service at 1st Free Will Baptist Church of Park Hills.

Background screening is an important component of the church’s commitment to providing a safe and responsible ministry environment. Depending on the position or responsibilities involved, screening reports may include criminal history, public records, and sex offender registry information.

This form may be used for prospective or current employees, volunteers, pastors, transportation personnel, safety team members, and others serving in positions requiring appropriate screening.

Please carefully review the disclosure before signing and dating the authorization.

By signing, the individual authorizes the church to obtain background screening reports for purposes of evaluating suitability for employment or ministry service, including additional screening during continued service when permitted by applicable law.

This authorization is separate from the Ministry Worker Application (FRM-002) and does not, by itself, constitute approval to begin serving.

Confidentiality: Completed authorization forms contain personal information and should be submitted securely to authorized church leadership. Do not upload completed forms to this public resource library.
