This agreement establishes the authorized use of church buildings, grounds, rooms, and equipment while helping ensure that individuals and organizations understand their responsibilities for supervision, safety, appropriate conduct, cleanup, and care of church property.
The form also provides space to document event-specific conditions, including approved access, insurance requirements, facility fees, deposits, custodial arrangements, equipment use, and any additional restrictions or requirements established by church leadership.

All applicable event-specific terms and conditions must be established before the agreement is signed.
The responsible individual or authorized group representative must review the agreement, acknowledge the conditions of facility use, and sign the document along with an authorized church representative.
Individuals and organizations using church facilities are expected to comply with the church’s Facilities Use and Building Stewardship Policy, observe applicable safety requirements, and return facilities and equipment to their approved condition.
Approval may be limited or cancelled in accordance with church policy. Damage, misuse, unusual cleaning, or failure to comply with approved conditions may result in applicable costs as provided by church policy and law.
