The Membership Application (FRM-012) is provided for individuals who desire to become members of 1st Free Will Baptist Church of Park Hills.

Church membership represents a meaningful commitment to Christ, to the fellowship of believers, and to participation in the life and mission of the local church.

This application helps individuals express their desire for membership while providing the church with the information necessary to consider and document their application.

The Four Membership Requirements #

The application addresses four requirements for membership at 1st Free Will Baptist Church:

1. Personal Faith in Jesus Christ

Applicants are asked whether they have committed their lives to Jesus Christ and trusted Him for salvation.

2. Believer’s Baptism by Immersion

Applicants are asked whether they have been baptized by immersion following their commitment to Jesus Christ. Those who have not yet been baptized may indicate their desire to be baptized at 1st Free Will Baptist Church.

3. Completion of the Connections Class

Applicants are asked to confirm completion of the church’s Connections class and provide the completion date.

4. Commitment to Church Membership

Applicants are asked whether they desire to abide by the 1st Free Will Baptist Church Membership Commitment.

Completing the Application #

The application also requests basic personal and contact information, including name, address, date of birth, telephone number, email address, and marital status.

Applicants are asked to sign and date the completed application. A separate section is reserved for recording church approval.

Submitting a Membership Application does not, by itself, constitute approval for church membership.

The application is part of the church’s membership process and is subject to the church’s established membership requirements and approval procedures.

Because the completed application contains personal information, it should be submitted directly to appropriate church leadership rather than posted or shared publicly.
