The Firearm Carry Authorization Record (FRM-011) is used to document firearm carry authorizations granted by the Lead Pastor of 1st Free Will Baptist Church of Park Hills, as well as applicable notifications from off-duty law enforcement officers under the church’s Firearms and Weapons Policy.

This form provides an official record of authorization decisions, applicable conditions, verification of qualifications, authorized locations or events, and any subsequent modifications, suspensions, or revocations.

The form may be used for approved Safety & Security Team members, law enforcement personnel, and other individuals considered for specific authorization under church policy.

The Lead Pastor retains authority to grant, limit, modify, suspend, or revoke firearm carry authorization in accordance with approved church policies.
